The following information packets should be filled out and brought to the physical or turned in when the athlete reports. If you have questions about either form, please contact one of the athletic trainers listed at the top of this page.
Below are the links to the student-athlete physical forms. If you are a new Faulkner athlete, you'll need to download the top form. If you are a returning athlete, download the second form.
All student-athletes are required to carry primary insurance. If you do not have insurance, the link listed below is available for your consideration. You are not require to purchase this policy, but a primary insurance carrier must be provided to the university prior to any physical activity. This policy, however, has been approved by the university to provide the essential coverage required for student-athletes
The link below has been provided for international student-athletes still needing primary insurance. The "Select" and "Budget" plans have been verified by the university to provide international athletes with the coverage needed. Players are not required to purchase this policies, but are required to have valid primary insurance prior to starting any physical activity. If an international athlete chooses an option other than the "Select" or "Budget" plan provided here, that information must be presented to the training staff. We strongly encourage athletes to consider these options when selecting a primary insurance policy.
Visiting Team Information
The training room is located in the Tine Davis Gymnasium for all sports other than football. The football training room is located in the football fieldhouse.
The training room will be open two hours prior to an athletic event.
A Certified Athletic Trainer will be at all home events.
Available modalities include: 4 channel e-stim, 1 ultrasound, moist-heat packs, ice bags, 1 large whirlpool & 1 extremity whirlpool(please give advance notice if this is needed). If there is anything else requested please contact the trainers above.
Ice, water, cups, and a guest box with biohazard bag & first aid supplies will be provided on the visiting bench. Injury ice and ice bags will be provided. Towels are available for Gymnasium Events only.
If a Certified Athletic Trainer is not traveling with the team, please provide written or verbal consent from the Certified Athletic Trainer for any pre-game treatment instructions of visiting athletes.
All visiting teams please make sure to bring a well-stocked kit for any pre-game taping/wrapping events. Also please make sure that there is exoclear for any post-game ice. We apologize, but our supplies are limited.

AED, crutches, splints and braces are available during competition.
